Verification of School Based Employment/Induction and Mentoring Form 

DESE requires this form in order to advance your license from Initial to Professional. The form documents that you have completed an induction and mentoring program and have completed 3 years of school-based employment.

1.  Print and complete the Verification of School Based Employment/Induction and Mentoring Form 

2.  Once completed, email (stacey.gottfried@mansfieldschools.com) or send via interoffice mail to Central Office, attn: Stacey

3.  The form will be signed and returned to you

4.  Upload the form to your ELAR Account

Public Service Loan Forgiveness (PSLF) Form

PSLFs can be emailed to Stacey.Gottfried@mansfieldschools.com.
