Provide the following documentation for residency and upload via Family Connection:

•  Residency Requirement Documentation

Print and complete the following forms and upload via Family Connection:

•  Transfer of Records Authorization Form 

•  Medicaid Authorization Form 

Collect the following documentation and upload via Family Connection:

•  Student Birth Certificate or Passport 

•  Parent/Guardian ID (license, passport or other Government Issued ID) 

•  School / Camp Form from your health provider including:

  •  Current Immunizations

  •  Completion of Physical (wellness check) within the last 13 months. 

  •  You do not need to send the full summary of your last visit.

•  Student Photo (Grades K - 5 only)

If applicable, please complete and upload via Family Connection

•  Residency Affidavit Form
  (ONLY IF you are living with another person who would be providing proof of residency)

•  Court Guardianship and/or Custody information 

• Copy of Current IEP or 504 Plan
